CPAP Troubleshooting: 20 Common Issues and How to Fix Them

Bookmark this page. You'll use it daily.

When patients call with problems, fast resolution keeps them on therapy. Here are the 20 issues you'll hear most often, with solutions that work.

Air Delivery Issues

1. "The air pressure feels too strong"

Possible causes:

  • Prescribed pressure higher than tolerable
  • Ramp feature not enabled
  • Patient not acclimated yet

Solutions:

  • Enable ramp (starts low, increases gradually)
  • Check if EPR/Flex is enabled (pressure relief on exhale)
  • If persistent, contact prescribing physician about pressure adjustment
  • Reassure: "Most patients adjust within 2-3 weeks"

2. "The air pressure feels too weak"

Possible causes:

  • Large mask leak reducing effective pressure
  • Machine not reaching prescribed pressure
  • Wrong mode (CPAP vs APAP)

Solutions:

  • Check mask seal and leak data
  • Verify machine settings match prescription
  • If APAP, check if max pressure is set appropriately
  • Review data for actual delivered pressure vs prescription

3. "The machine seems to stop working during the night"

Possible causes:

  • Auto-off feature (no breathing detected)
  • Patient removing mask unconsciously
  • Power interruption

Solutions:

  • Check mask fit (large leak may trigger auto-off)
  • Check power connection and outlet
  • Review data for pattern of usage gaps
  • Ask about sleep position and mask displacement

4. "Water is getting into my mask/tubing"

Possible causes:

  • Rainout (condensation in tube)
  • Humidifier overfilled
  • Room temperature colder than machine

Solutions:

  • Use heated tubing (best fix)
  • Lower humidity setting
  • Position machine lower than bed
  • Insulate tubing with cloth cover
  • Adjust room temperature

5. "The machine is making a whistling sound"

Possible causes:

  • Mask leak
  • Tubing connection loose
  • Filter clogged or improperly seated

Solutions:

  • Check all connections (mask, tubing, machine)
  • Inspect and replace filters if needed
  • Check mask cushion for tears or debris
  • Ensure mask is properly sized

Mask Comfort Issues

6. "The mask hurts my nose/face"

Possible causes:

  • Mask too tight
  • Wrong size
  • Extended use without breaks

Solutions:

  • Loosen straps (two-finger rule)
  • Try different size cushion
  • Use mask liner or gel pad
  • Consider different mask style
  • Ensure face is clean before wear (oil affects seal)

7. "I'm getting red marks on my face"

Possible causes:

  • Over-tightened straps
  • Allergic reaction to silicone
  • Mask not properly positioned

Solutions:

  • Adjust strap tension
  • Reposition mask
  • Try hypoallergenic mask liner
  • Switch to different mask material
  • Apply barrier cream (non-petroleum)

8. "Air is leaking into my eyes"

Possible causes:

  • Mask positioned too high
  • Top strap too loose
  • Wrong cushion size

Solutions:

  • Lower mask position on nose
  • Adjust top strap first
  • Try smaller cushion size
  • Check for cushion wear/deformation

9. "The mask is leaving dents in my face"

Possible causes:

  • Mask too tight
  • Headgear not properly adjusted
  • Wrong mask style for face shape

Solutions:

  • Loosen all straps
  • Redistribute headgear pressure
  • Consider memory foam cushion options
  • Switch to lower-profile mask style

10. "I can't find a comfortable position with the mask"

Possible causes:

  • Mask bulk interfering with pillow
  • Tubing pulling or tangling
  • Patient is active sleeper

Solutions:

  • Try CPAP pillow (cutouts for mask)
  • Use tubing management clip
  • Position tubing over headboard
  • Consider low-profile or minimal-contact mask
  • Try different mask style for sleep position

Dryness and Congestion Issues

11. "My mouth is really dry in the morning"

Possible causes:

  • Mouth breathing with nasal mask
  • Insufficient humidity
  • Mouth leak

Solutions:

  • Add chin strap (first line for nasal masks)
  • Increase humidity level
  • Try full face mask if chin strap fails
  • Check for room humidity issues
  • Hydrate well before bed

12. "My nose is dry/burning"

Possible causes:

  • Humidity too low
  • Nasal passage irritation
  • Allergies or cold

Solutions:

  • Increase humidity gradually
  • Use nasal saline spray before bed
  • Check heated tubing temperature
  • Consider nasal moisturizing gel
  • Rule out illness or allergies

13. "I'm waking up with a bloody nose"

Possible causes:

  • Severe nasal dryness
  • Physical irritation from mask
  • Low ambient humidity

Solutions:

  • Max humidity setting + heated tubing
  • Saline rinse before bed
  • Petroleum-free nasal moisturizer
  • Check mask fit (not pushing on septum)
  • Room humidifier if ambient <30%

14. "I'm congested and can't use my CPAP"

Possible causes:

  • Cold or allergies
  • Deviated septum
  • Chronic sinusitis

Solutions:

  • Short-term: Nasal decongestant spray (max 3 days)
  • Saline rinse before bed
  • Increase humidity
  • Full face mask as backup during illness
  • Refer to ENT if chronic

Stomach and Digestive Issues

15. "I'm waking up feeling bloated/gassy"

Possible causes:

  • Aerophagia (swallowing air)
  • Pressure too high
  • Patient sleeping position

Solutions:

  • Elevate head of bed
  • Try EPR/Flex pressure relief
  • Request pressure titration if severe
  • Sleep on side instead of back
  • Avoid eating close to bedtime

Sleep Quality Issues

16. "I can't fall asleep with the mask on"

Possible causes:

  • Anxiety about mask/machine
  • Not acclimated
  • Discomfort

Solutions:

  • Wear mask while watching TV to acclimate
  • Use ramp feature (start low pressure)
  • Relaxation techniques before bed
  • Ensure mask is comfortable before lying down
  • Give it 2-3 weeks (adjustment period is normal)

17. "I keep taking the mask off during the night"

Possible causes:

  • Claustrophobia
  • Leak causing arousal
  • Pressure discomfort

Solutions:

  • Try different, less restrictive mask
  • Ensure seal is good (reduce leak-related arousals)
  • Ramp feature to fall asleep at lower pressure
  • Rule out other sleep disturbances
  • Consider sleep aid during adjustment (physician approval)

18. "I still feel tired even though I'm using the CPAP"

Possible causes:

  • Inadequate therapy (check AHI)
  • Other sleep disorders
  • Medical conditions
  • Insufficient sleep time

Solutions:

  • Review compliance data (are they using it enough?)
  • Check AHI - should be <5 (if not, therapy not effective)
  • Review leak data (high leak = reduced effectiveness)
  • Consider other causes (depression, thyroid, other sleep disorders)
  • Refer back to sleep physician if AHI controlled but symptoms persist

Machine and Equipment Issues

19. "My machine smells bad"

Possible causes:

  • Mold/mildew in humidifier
  • Dirty filters
  • Bacteria in tubing

Solutions:

  • Clean water chamber daily
  • Replace water daily (don't reuse)
  • Clean tubing weekly with mild soap
  • Replace filters per schedule
  • Use distilled water only

20. "The machine is louder than it used to be"

Possible causes:

  • Filter clog
  • Motor wear
  • Leak causing turbulence
  • Incorrect machine position

Solutions:

  • Check and replace filters
  • Ensure all connections are secure
  • Check for mask/tube damage
  • Ensure air intake isn't blocked
  • If motor issue, machine may need service/replacement
