Morning Headaches with CPAP? Causes and Solutions

CPAP is supposed to make mornings better. So why do you have a headache?

Morning headaches have several possible causes. Let's figure yours out.

Possible Causes

Mask Too Tight

Signs:

  • Headache focused where straps sit
  • Forehead or temple pain
  • Marks or indentations on face

Fix: Loosen headgear. The mask should seal gently.

Sinus Pressure

Signs:

  • Pain around eyes, cheeks, forehead
  • Worse when bending over
  • Nasal congestion too

Fix: Address congestion (see nasal congestion article), saline rinse, consider allergy treatment

Pressure Issues

Signs:

  • Waking with feeling of "head pressure"
  • Headache plus other symptoms (bloating, ear popping)

Fix: Talk to provider about pressure settings

CO2 Retention (Rare)

Signs:

  • Severe morning headaches
  • Excessive sleepiness despite CPAP
  • History of lung disease

Fix: See your doctor. May need different equipment.

Dehydration

Signs:

  • Dry mouth also
  • Not drinking enough water
  • Headache improves with water

Fix: Drink more water throughout the day, ensure humidifier is working

Sleep Apnea Not Fully Treated

Signs:

  • Still snoring
  • Still tired
  • Partner notices breathing issues

Fix: Review CPAP data with provider. May need pressure adjustment.

Step-by-Step Troubleshooting

  1. Check mask tension: Can you slip a finger under straps? If not, too tight.
  1. Review humidification: Dry air causes sinus headaches. Turn up humidity.
  1. Consider congestion: Stuffed nose? Treat it (see congestion solutions).
  1. Track patterns: Same every day? Only certain days? Write it down.
  1. Check your data: Is therapy working? High AHI despite CPAP?

When to See Your Doctor

Get medical attention if:

  • Headaches are severe
  • Accompanied by vision changes
  • New and different from past headaches
  • Not improving with adjustments

They may:

  • Review your CPAP data
  • Check for other conditions
  • Adjust treatment

What Often Helps

Most common solutions:

  • Loosening headgear
  • Increasing humidity
  • Treating nasal congestion
  • Staying hydrated
  • Ensuring CPAP is working properly

Headaches from Sleep Apnea

Remember: Untreated sleep apnea causes morning headaches too.

Before CPAP: Low oxygen, disrupted sleep = headaches

With CPAP: Usually fewer headaches

If headaches started with CPAP: Probably equipment-related (fixable!)

If headaches continue despite CPAP: Therapy may need optimization
